Family Outcomes Survey
Your voice matters!
As your family's Early Intervention journey comes to an end, you may be invited to complete the New York State Family Outcomes Survey.
This short survey gives you the chance to share your family's experience with the Early Intervention Program. Your feedback helps the New York State Department of Health learn what is working well for families and where improvements can be made.
The survey asks about your family's experience and whether Early Intervention helped you:
- Understand your child's strengths and needs.
- Feel more confident helping your child learn and grow.
- Know your rights and speak up for your child.
- Find the services and resources your family needed.
The survey takes about 10 minutes to complete, and your responses are confidential. Your answers are combined with those from other families across New York State and are never shared with your name.
Every family's story is important. By taking a few minutes to complete the survey, you're helping improve Early Intervention services for future children and families.
